We here at Hyatt Family Dental understand that it can be difficult to budget for dental work with rising costs and no dental insurance. With that in mind, we are happy to announce that we are offering an in-house membership plan to assist you with your dental care. Good dental hygiene is an important aspect of maintaining your overall health. This alternative to dental insurance will save you money while giving you the same great service that you expect from Hyatt Family Dental.

This is not Dental Insurance. This is only available at Hyatt Family Dental and is not valid at any other dental offices. The membership fee is available to be paid either monthly, or annually, and automatically renews every 365 days. We require a month’s notice to be given to cancel the automatic renewal of the plan.

With this membership, there are:

• No waiting periods

• No annual maximums

• No age limitations

• No deductibles

• No pre-determinations

• No claim forms

You will receive:

• Two preventative prophylaxis procedures per year (“cleanings”)

• Two periodic dental examinations per year

• Two fluoride treatments with no age restrictions per year

• Any x-rays associated with preventative visits (1 set of bite wings per year, 1 panoramic x-ray every 5 years, any needed Pas)

• Oral cancer screening, blood pressure reading, oral hygiene instruction, and any other services typically included in a preventative appointment

• One emergency examination per year with any necessary radiographs (this does not include any treatment needed due to the emergency)

• 20 % discount on all other restorative dental services.

For all recommended procedures, there will be a treatment plan provided with all services and fees shown. The discounted rates and total fees will be provided prior to treatment. No payment plans or outside financing are available with the Dental Membership Plan.

Full payment must be made at time of service to receive a 15% discount.
